- A completed New Patient Form
IMPORTANT NOTE ABOUT FIRST APPOINTMENTS
It is our policy, without exception, that every first appointment will be scheduled with a provider, regardless of the reason for the appointment. A provider will have reviewed the patient’s records prior to the appointment; medical history and any concerns will be addressed at the appointment. This is for the safety of all. Please be aware that this visit will trigger a charge that could apply to your deductible, depending on your insurance and deductible. You may want to check with your insurance first, so that you are aware of potential charges in advance.
INSURANCE AND PAYMENT OPTIONS
We accept most of the commonly available forms of health insurance (HMO, PPO, Private Insurance) and Medicaid (Tufts Health Together with Boston Children’s ACO only). For balances after insurance and for co-pays, we accept a variety of payment options, including cash, check, and credit cards. Unfortunately we do not accept Self-Pay as a form of payment except in special circumstances, when approved by the practice manager. Lastly, our practice manager will be happy to set up a payment plan for your balance should you want one. Please contact our office if you have any questions about insurance or payment options.
